This portfolio section highlights my capstone work, professional evaluations, and key achievements as a DNP-prepared nurse leader, including implementation of evidence-based quality improvement initiatives, preceptor and leadership feedback, and recognition for excellence in clinical practice and team collaboration. Capstone
Led a Doctor of Nursing Practice quality‑improvement project, “Workflow Wizardry: Mastering Denosumab Administration,” to design, implement, and evaluate an evidence‑based, nurse‑driven workflow for denosumab biosimilar injections at Sanford Health Family Medicine North Fargo Clinic. Conducted a comprehensive literature review, applied Orlando’s Nursing Process Theory and Rogers’ Diffusion of Innovation to guide change, and mapped a full clinical workflow including recall reports, pre‑visit and visit‑day processes, EMR dot phrases, and safety checks for labs and contraindications. Collaborated with nursing leadership, providers, data analytics, IT, and a nurse champion to build EMR tools, develop staff education materials, and obtain IRB approval from both the university and health system.
During a six‑week implementation window, educated 100% of available nursing staff and providers, achieving very high self‑reported gains in knowledge, confidence, and competence related to denosumab administration, patient counseling, and workflow expectations. Early outcome data showed that the new recall report successfully identified 20 patients with active denosumab indications, including those overdue or approaching due dates, prompting appropriate follow‑up actions such as scheduling, referrals, and order placement. Chart audits and workflow surveys revealed strong gains in diagnosis documentation, prior authorization completion, and perceived patient safety, while also uncovering gaps in lab‑monitoring documentation, dot‑phrase use, and perceived time efficiency, leading to targeted recommendations for extending the implementation period, simplifying EMR documentation, and strengthening leadership support for sustainability.
Positioned the workflow for spread across additional Sanford Family Medicine clinics and specialty departments, with sustainability strategies that integrate the process into onboarding, annual competencies, EMR quick links, and ongoing quality‑review structures. Overall, the project demonstrated how a DNP‑prepared nurse can lead interdisciplinary teams, leverage EMR and data tools, and translate evidence into a scalable, low‑cost intervention to enhance osteoporosis care and reduce fracture risk in a high‑need population.

Professional Evaluations/Feedback
2025 Evaluation:
Very focused on quality care; both for the patient and raising the expectations of the nursing staff. Skillfull mentor; able to demonstrate and hold staff to expectations of licensure as well as provide helpful feedback, coaching, and follow up as needed. Strong SAFE skills focus throughout leadership of clinic and with staff and providers through demonstration, sharing of resources, education, and follow up. Very "hands-on" leadership with staff; demonstrating the "how" and the "why" for effectiveness as well as building supportive relationship throughout learning.
Nomination for Transformational Leadership Nurse Award (2023):
My Granddaughter, in college, texted me recently to ask my opinion and in my years of nursing experience, what I felt were good qualities in a leader. I listed the following to her: Dependable, knowledgeable, reliable, honest, trustworthy, kind, approachable, leads by example, nonjudgemental and willing to help out. As I was sending the text to her, I thought to myself this is Christina (Tina) Hoff. I have had the privilege of working with Tina for several years now and have watched her take on more nursing responsibility and leadership roles all while furthering her own education. She has used her enthusiasm and professionalism to help build our Integrative Medicine Department and now providing leadership at South Pointe Family Medicine. She has an interest in all aspects of providing the best possible nursing care and helping those she works with to build this within each of them. She genuinely cares for our patients, providers and especially those of us staff workers who are fortunate enough to have her as our supervisor. I believe Tina is very deserving of this recognition.
Nomination for Employee of the Year (2020):
Tina Hoff is a Sanford RN Team Lead Supervisor for Integrative Health, but she is more than a Nurse. She is a positive, compassionate, caring, and patient center-driven Leader, who holds several unique skillsets that Sanford is lucky to have someone as wonderful and dedicated to her Team and Patients as Tina. Tina is a dedicated and hardworking leader that helps keep our Department not only running smoothly but to have out department positively stand out. We are a True Team and Tina helps mold the environment. Tina is always making sure that the needs of not only the patients, but that the entire department needs are met. She will lead us in daily yoga after we communicate any updates so we are all on the same page and start our day with Self-Care and a positive message. She guides us in stretching our body and our minds and thus opening up the space to be totally present for our patients. She supports her staff to reach their goals and alerts us to opportunities to excel in our role and discusses with us areas of our interests and goals and is supportive of obtaining the time to attend these trainings as well as supports our growth. Tina has shown all the Sanford Values and is a true Leader who is part of the Team with you. She is by your Side to aide wherever she can. She will help us out when we are busy in the clinic by rooming patients, she is always helping out the phone nurses with triaging and aiding in the phone messages and patient questions. She also will help out other departments without being asked for help. She will do all this as we are all part of the Same Team. Sanford Health and she is a Prime example of going the extra mile each and every day. She cares about her patients, her staff, her providers, her department, the Sanford Family, and the community as a whole. We are thankful to have such a wonderful example of a true leader who is so giving and has a healing presence in herself. Please consider Tina Hoff for the Employee of the Year as she is more than deserving of such a great accomplishment and acknowledgment.

It was an absolute pleasure having Tina for the time she was at our clinic. She is an experienced nurse and it shows that she has a wide range of experience working in the medical field. I strongly believe that because of her diverse background she will make an exceptional provider. She displays a personable and nurturing environment while also maintaining professionalism and healthy boundaries, this is key to a healthy patient-provider relationship.
